IBF Smart Tag won't activate in Word

I have created an IBF solution with smarttag. When I run in debug it works
as expected. However, if I publish the metadata and then try and open Word,
the smart tag does not appear at all. I have deleted my cache (every
possible way!). I cannot find any permissions issues, as all the dll's are
fully trusted and signed with a key.

Is there ANY way to debug this sort of thing, or does anyone have any
suggestions as to how I might find out what the problem is?
