workFlow stops working

hi all

i have a problem regarding my workflow that i've created.

it stops working when an item is changed.

i've have checked the textbox that says "automatic start this workflow whenever a item has changed"

the workflow runs for the first time but when the item is changed again, its stops working with an error msg of "Error updating a list item"

would like to know what i have done wrong.
